By signing below, I agree to the following conditions for riding the Andover-Whitesville spectator bus:
· Conduct: I agree to represent the Andover-Whitesville community positively and adhere to all school district policies regarding conduct on school property/vehicles (including the prohibition of alcohol, tobacco, and nicotine products).
· Liability: I understand that Andover-Whitesville School is not responsible for the loss or damage of personal property.
· Medical: In the event of a medical emergency, I authorize the trip supervisor to seek emergency medical care if my designated contact cannot be reached.
· Return Trip: The bus will depart Blue Cross Arena shortly after the final whistle. Passengers are responsible for being at the designated pickup spot on time.
