ANDOVER CENTRAL SCHOOL REGISTRATION FORMS
Please find below registration forms for students entering pre-k, kindergarten, elementary grades 1-6 and Jr/Sr high school grades 7-12.
If you are new to the district, a meeting with our principal will be set-up when you bring in the paperwork to the Guidance Office, please bring all required documents with you at this time.
When completing registration forms please be sure to also complete the language questionnare, release of health information, and release of information also.
