To obtain working papers, please complete each section of the form on the left side.
The Physical Fitness Certification may be completed by the school nurse, provided your child's most recent physical examination performed by their physician was completed within the past 12 months.
Proof of age is also required. Acceptable documents include:
- State-issued photo ID
- Driver's license
- Passport
- Birth certificate
If your child is a registered student at ACS, we may already have proof of age on file.
Please note that once your child turns 16, they must receive an updated working papers card, even if they already have one.
Working paper colors by age:
- Ages 14–15: Blue card
- Ages 16–17 (currently attending school): Green card
- Ages 16–17 (not attending school): Salmon card
