Code of Athletics Acknowledgement and Sports Participation

Attached is the Code of Athletics as well as the Sudden Cardiac Arrest Informational letter. Please complete the Form attached to Acknowledge your receipt, understanding and agreement with the Code of Athletics, as well as Emergency Contact Information for this school year.
Note: Completion of the Form is only required if your student-athlete is planning on participating in a Fall Sport.
